Fruition Community Support Specialist
Description

The Fruition Community Support Specialist will provide and coordinate services that are an integral part of helping LIFECORE Health Group’s consumers by providing holistic family driven, person-centered approach. The CSS will provide optimal care through the use of evidenced based practices. Both traditional and nontraditional supports will be key to optimal achievement of program goals and to allow for a seamless system of care 

Responsibilities
  • Provide conscientious mental health care services to children with serious/severe emotional disturbance
  • Participate in all training required by the Department of Mental Health and LIFECORE Health Group so as to maintain current credentials
  • Adhere to quality assurance guidelines
  • Actively collaborate with other systems’ staff regarding assigned individuals and their families
  • Provide all required documentation pertinent to provision of services
  • Seek culturally competent services and supports to meet the unique cultural needs of youth and family
  • Attend Local MAP team Meetings
  • Other duties as assigned
Qualifications
  • Bachelor’s degree in Psychology or Social Work
  • Valid Driver’s License
  • Valid Car Insurance
  • Reliable Transportation
