The Care Coordinator will help lead LIFECORE Health Group to excellence by ensuring integrated care. They will make sure that nursing services are provided and coordination of behavioral health services, primary care services and other internal and/or external services is maintained for all clients.
· Provide nursing services that maintain the quality measures and goals of the CCBHC Program. · Take vitals and record them for use to further primary care services and integrate care. · Research and implement best practices for nursing services. · Medication management and follow up with providers to make sure that all clients enrolled have access to the medications they need. · Maintain documentation necessary with regulatory agencies standards. · Outreach activities/networking for medical care referrals internally and externally to ensure the whole healthcare approach of the CCBHC. · Work with internal providers and triage staff to make sure all clients are assessed appropriately and screened for appropriate placement and services. · Other duties as assigned.
· Licensed Practical Nurse · Experience in using computer software and EMR proficiency. · Demonstrates ability to use data and reporting to drive clinical decision making and maintain health goals for the populations served. · Excellent interpersonal skills that allow the incumbent to build strong relationships both within the agency and with vital outside interests and with clients. · Proficient with Microsoft Excel and Word.
