Recovery Outreach Worker
Description

To enhance follow up care to individuals transitioning from crisis or in-patient treatment to recovery.  The Recovery Outreach Worker will follow up with clients post discharge to prevent relapse. 

Responsibilities
  • Conduct in-person follow-up visits or telephone check-ins with recently discharged clients 
  • Encourage clients in relapse prevention and life skills as needed 
  • Collect follow-up GPRA surveys 
  • Maintain documentation of check-ins and related data 
  • Provide monthly and year end reports to DMH 
  • Perform other duties as assigned    
Qualifications
  • High School Diploma or GED 
  • Experience in computer software systems and EMR proficient 
  • A minimum of one (1) year of related work experience  
  • A minimum of one (1) year non-abusive use of substances 
  • A valid driver’s license and insurance
