PACT Therapist

The purpose of Program of Assertive Community Treatment Therapist is to provide rehabilitative and support services in the community as a member of a multi-disciplinary team, in order to reduce hospitalizations, assist with the transition from the hospital to the community, and/or improve the quality of life for individuals with severe and persistent mental illness in accordance with the PACT Model

The PACT Therapist will help lead Lifecore's Person Assertive Community Treatment program to excellence by providing clinical services within an outpatient setting for those dually diagnosed with mental health & substance use disorders; assisting and educating the client and their families with mental health disorders, medication management, linking to community resources, court system, incarceration and a myriad of wrap around services provided by the PACT Teams.


Duties shall include but not be limited to the following:

  • Provides evidence-based individual, family, and group clinical mental health therapy services and crisis response services to clients with severe and persistent mental illness. 
  • Provides community support and rehabilitative services to individuals with severe and persistent mental illness including but not limited to assistance with: personal hygiene, meeting nutritional needs, finding and maintaining safe/affordable housing, accessing reliable transportation, maintaining medication regimen, and accessing community services. 
  • Participates as a member of a daily organization staffing for the purpose of clinical consultation, utilization review, documentation verification and an exchange of information regarding consumers.  
  • Performs shift management and provides on-call crisis intervention covering nighttime and weekend hours, along with other staff members. 
  • Maintains all clinical records timely and appropriately, as required by agency and program policies, funding sources and accreditation authorities.   
  • Completes all required forms and maintains in a manner consistent with PACT requirements.  
  • Meets or exceeds the direct service productivity standard as established and reviewed by the PACT Supervisor/Director.  
  • Performs other duties as required and/or assigned by the PACT Supervisor/Director in accordance with DMH and LIFECORE standards. 
  • Promote Lifecore's PACT program in a professional manner at community events and networking opportunities.
  • Provide clinical services that include individual, family and group therapy to individuals and families with mental, emotional, physical problems and/or substance abuse.
  • Serve clients by providing and advocating for any effective services that will aid the client in daily living.
  • Work with the treatment team to develop a treatment/recovery plan based on each client’s identified goals.
  • Maintain a working knowledge of current trends and developments in the mental health field and attend continuing education seminars and other training when offered and authorized.
  • Must maintain 25 hours of billing productivity per week.
  • Valid MS driver’s license and clean driving record required · Proof of valid automobile insurance
  • One year work experience with SMI population preferred
  • Obtain DMH certification within 90 days of hire
  • Knowledge of therapeutic modalities, diagnosing, development of treatment plans and the ability to facilitate them using established standardized mental health processes.
  • Knowledge of the signs and symptoms of mental illness and the ability to assist the client to address symptoms.
  • Master’s Degree in behavioral health field (counseling, psychology)
  • Licensed/Certified or eligible.
